USCIS Civil Surgeon Medical Exam: Process, Tests, and Cost Explained 

You’ve found a civil surgeon and booked the appointment. Now, what actually happens during the USCIS medical exam? Understanding the USCIS medical exam process helps you prepare documents, avoid delays, and estimate the total cost.

The USCIS medical exam is not a typical check-up. Instead, it is a targeted screening designed to meet the requirements of the Department of Homeland Security. What Happens During the Exam?

The appointment usually lasts 30 to 60 minutes. The process typically involves: 

  • Paperwork Review: The civil surgeon will review your Form I-693 and your vaccination records.

  • Tuberculosis (TB) Screening: For applicants aged 2 and older. Usually done via an Interferon-Gamma Release Assay (IGRA) blood test. 
  • Syphilis Test: Required for applicants aged 15 and older.
  • Gonorrhoea Test: Required for applicants aged 15 and older. 

Required Vaccinations: 

You must be up to date on the following (unless waived for medical or religious reasons): 

  • Mumps, Measles, and Rubella (MMR)
  • Tetanus, Diphtheria, Pertussis (Tdap)
  • Varicella (Chickenpox) 
  • Hepatitis A & B 
  • Influenza (during flu season) 
  • Pneumococcal (for those over 65) 

Note: If you are missing a vaccine, the civil surgeon can often administer it during the appointment. 

Form I-693 Completion 

Once the tests come back negative (or positive with appropriate treatment completed) and vaccinations are verified, the civil surgeon will: 

  1. Complete and sign Form I-693
  2. Place the form in a sealed envelope
  3. Give you the envelope (or send it directly to USCIS if electronic processing is available).
