USCIS Civil Surgeon Explained: Who They Are and What They Do

If you are applying for a green card (adjustment of status) or certain visas in the United States, you’ve likely encountered the term “USCIS Civil Surgeon.” While it sounds like a specific medical specialty, it is actually a legal designation. 

Understanding who these doctors are and why the U.S. government requires them is the first step to a successful immigration medical examination. 

What is a USCIS Civil Surgeon?

A USCIS civil surgeon is a licensed physician authorized by the U.S. Citizenship and Immigration Services (USCIS) to conduct medical examinations for immigration purposes. They act as a bridge between your personal health and the public health requirements of the United States. 

Unlike a regular doctor who focuses solely on treating illnesses, a civil surgeon’s role is to ensure that immigrants are not inadmissible to the U.S. due to health-related grounds, such as vaccine-preventable diseases or certain medical conditions. 

Why Does USCIS Authorize Specific Doctors? 

The government does not allow just any doctor to perform the immigration medical exam. USCIS authorizes specific physicians to ensure the following: 

  • Standardization: Every immigrant undergoes the same testing criteria (based on CDC technical instructions). 
  • Accountability: Civil surgeons are trained to properly complete Form I-693, Report of Immigration Medical Examination and Vaccination Record
  • Integrity: They understand the legal implications of the exam and are authorized to certify that an applicant meets the health requirements for entry into the U.S.

Civil Surgeon vs. Regular Doctor: What’s the Difference?

The civil surgeon is the only person who can complete the Form I-693. If you go to a regular doctor for a physical, USCIS will reject the paperwork. 

The civil surgeon is responsible for: 

  1. Verifying Immunizations: Ensuring you have all required vaccines (MMR, Hepatitis B, flu, etc.). 
  1. Conducting Screening Tests: Administering blood tests for syphilis and (for applicants over 2 years old) tuberculosis (TB) via blood test or skin test. 
  1. Reviewing Mental Health: Determining if you have a physical or mental disorder associated with harmful behaviour. 

Once the exam is complete, the civil surgeon will sign the form and seal it. This sealed envelope is a critical piece of your immigration application.
